Student Health


At Jefferson School District, the health of our students is one of our utmost concerns. Consistent school attendance is encouraged to ensure your student benefits from their educational experience.  In the event your student needs medication during school hours, please complete the Authorization for Self-Administration form below and return it to the students school.

If you have questions regarding student health management for conditions such as Asthma, Diabetes, Heart Condition, Seizure or Allergy (requiring an epi-pen), please fill out the Questions or Comments information and the school nurse will be notified.
